Abbottabad: The Surgery Department at Ayub Teaching Hospital has achieved a major medical milestone by successfully performing a rare and complex laparoscopic cholecystectomy on a patient diagnosed with Situs Inversus Totalis—a congenital condition in which internal organs are arranged in a mirror image of their normal positions.
The delicate surgery was led by Assistant Professor Dr. Bahrrom, an expert in general, laparoscopic, and bariatric surgery, along with his experienced medical team. The 48-year-old male patient from Mansehra, who had previously undergone coronary artery bypass surgery in 2017 and was diabetic, was admitted to the hospital due to gallstones and severe inflammation of the gallbladder.
Given the patient’s rare anatomical condition and medical complications, several surgeons had earlier declined to operate. However, the Ayub Teaching Hospital team accepted the challenge and, after detailed planning, successfully conducted the surgery in Operation Theatre No. 1, Block B.

Dr. Bahrrom said the gallbladder was extremely swollen and difficult to locate due to the reversed organ arrangement. “It required utmost precision, teamwork, and technical expertise, but the operation went smoothly,” he added.
The patient made a quick recovery and was discharged the following day in good health.
The hospital administration hailed the successful procedure as a testament to the professional excellence, advanced surgical capabilities, and modern facilities available at Ayub Teaching Hospital.
Dr. Bahrrom expressed gratitude to his entire surgical and anesthesia team — Dr. Naeem, Dr. Usama, Usman (Anesthesia Technician), Dr. Irfan Malik, Dr. Saddam, Dr. Misbah, Lady Surgeon Dr. Farwa, Waqar, and Ubaidullah — for their dedicated cooperation and contribution to the success of the rare case.
